bound, hereby, for myself and anyone acting on my behalf, release any and
all claims for damages I may have against the City of Jacksonville Beach,
Festival organizers, City of Jacksonville, Performance MultiSports,
the race directors, race staff, volunteers, and any sponsors and
their representatives, successors and assigns for any and all injuries and
death suffered by me in said event. I attest and verify that I know that a
running competition can be a potentially hazardous activity. However, I
have sufficiently trained for the completion of this event and know that I
am participating at my own risk. I am also aware of the dangers of
exercising or competing in hot weather and humidity. I have read this
waiver of liability and accept the risks and responsibility associated
with entering this event. I hereby grant permission for the free use of my
name and picture in any broadcast brochure, or account of this event. I
understand that by not pre-registering by the cut-off date listed online,
I may not be able to get a souvenir T-shirt from this event.

Start on the beach in front of the
Life Saving Station, at Beach Boulevard, in Jacksonville Beach, FL
DATE & TIME:
4 April 2009 -The
5K Run & 2.5K Walk start at 9:00AM (for the tide)
COURSE: Accurate,
measured 5-Kilometer (3.1 mi.) course on the flat, smooth beach, starting
and finishing at the Red Cross Life Saving Station, Jax Beach. Aid
stations with water at Miles 1.5 and at the finish. Markers at each mile
and at 1/2 mile to go. Walk is 2.5K (1.55 mile) on the beach.
AWARDS: Awards
to top Overall and Masters, male & female, plus top three winners in
each age group: 13 & under, 14-19, 20-25, 26-29, 30-34, 35-39, 40-44,
45-49, 50-54, 55-59, 60-64, 65-69, 70 & up. -

REGISTRATION:
Register
by mail until 30 March, by
sending completed entry form and fee to: Performance MultiSports, 365
Charlemagne Circle, Ponte Vedra Beach, FL 32082.
AFTER 4/2: Register on Race
Day starting an hour before race starting time at race site.
ENTRY FEES: Early
(postmarked before 3/15) = $15.00 --- After 3/15 through4/1 = $17.00 --- From
4/1 through Day
of Event = $20.00

PACKET PICK-UP & LATE ENTRY: race
packet pick-up and race-day registration at the Lifesaving Station at the east end of
Beach Boulevard on race day, beginning an hour before race start.
